Service Coordination
Support Services
At Baunta Apartments, HUD guidelines specify that residents are considered completely independent. However, the reality is that as the senior population ages, the need for services – increased assistance with their activities of daily living (ambulating, bathing, dressing, eating, grooming, and home care) – becomes more pressing.
Most of these services are available through various community resources. To assist residents in accessing these services, Sayre Christian Village employs a full-time Service Coordinator, who establishes links to community agencies and services providers.
Establishing these links allows the Service Coordinator to arrange:
• Housekeeping services
• Meal services
• Transportation and Escort services
• Financial management assistance
• Counseling and support
• Physical/Occupational therapy
• Medication reminders
• Additional services depending on the resident’s need
The Service Coordinator also performs daily tasks to assist Residents, such as:
• Providing information and help with Social Security,
Medicare, Medicaid, supplemental insurances, food
stamps, energy assistance programs, and etc.
• Helping residents interpret mail, complete paperwork,
arrange utility, phone and payment schedules
• Arrange for a senior companion or volunteers to visit
with residents
• Implementing mobile health services
• Working with a resident’s health care provider to
ensure their needs are being met
• Organize support groups that deal with topics such
as grief, illness, depression, loneliness, vision or
hearing impairment, and diabetes.
The Service Coordinator’s primary goal is making it possible for residents to age in place by connecting them to resources available within the community. The result is improved resident quality of life and decreased premature or unnecessary institutionalization.
The Service Coordinator is available to serve ALL residents of Baunta
